New Physical Exam Forms for Upper School Students

As of June 1, 2024, the NJ Department of Education has updated these forms. The new forms have been updated in your child’s Magnus account and can also be downloaded here.

 

In addition, any student participating in athletics must have a physical within one year of the first practice of the season. As of June 1, 2024, The NJ Department of Education has updated the Student Athlete Pre-Participation Evaluation (PPE) Forms, which include the Health History, the Physical Examination Form, and the Medical Eligibility Form.

 

All new physicals submitted this summer and going forward should be using these new forms. While the forms may state that they are not to be submitted to the schools, GOA does require all these forms (health history, physical exam, and medical eligibility form) be submitted via Magnus annually. Partial form submission will not be accepted. Feel free to reach out to Nurse Ilena with any questions.
